Ticketing Form Link

You can setup a link to your ticketing form from your website.

First go to Setup, Support, Departments and add your departments such as "Support" and "Billing"

Use this link

https://YOURTENANTID/mspcrm.net/forms/ticket?col=col-md-8+col-md-offset-2

If you need to add more questions, go to Setup, Custom Fields.
